All appointments will now be scheduled through your therapist, who will be able to enter them into the Spring Health calendar.  Also, you can contact Rooted Vines Counseling at the main number (518) 763-1700 by call or text to schedule an appointment.  

Even though you are connected to us through Spring Health, you're still part of the Rooted Vines Counseling family.  Below, you will find a link to applicable practice forms for your review and reference.  If there are any questions about these forms, please speak to Shannon or your therapist.  

Telehealth appointments will be conducted using Spring Health's HIPAA compliant platform.  

Please find a confidential location for your telehealth appointment.  Please do not conduct your telehealth appointment while driving.  

We are located in Socha Plaza at 

123 Saratoga Rd. in Glenville, NY 12302.  

In person appointments are available and encouraged.  Come a little early and help yourself to a cup of coffee or tea.  

If you need to cancel for any reason, please do so at least 24 hours before your appointment time.  Do so by either emailing your therapist or Shannon or calling/texting 518-763-1700.

Appointments cancelled less than 24 hours in advance will be subject to a no-show fee.  This fee is set by Spring Health and varies based on benefit.  Please ensure that you reach out to your therapist or Shannon to cancel your appointment.  

Copays will be billed by Spring Health, when applicable.  If there are any issues with billing for services or copayments, please contact Spring Health (1-844-931-4465) or your insurance company.    

Once you're enrolled, you can speak to your therapist about their preferred way of communication and vice versa.  Communication options include:

• by portal secure messaging*
